Thank you for thinking of a gift to The Denis Theatre Foundation as a great way to honor a friend or family member.

Please fill out the form below to let us know who you are and in whose name you are making your gift. Then you can either make a secure online payment with a credit card or PayPal or you can send your donation through the mail.

When we receive your donation we will match it with this form and send an acknowledgement to you, as well as notification to the honoree that you've made a gift in their name.
